Construction Inspection Request

Please use this form to request an inspection related to a new construction or renovation project. Inspection requests for other inspection types can be made here. You must submit a request at least 24 hours prior to an inspection on Tuesday – Friday with the exception of recognized holidays. Inspection requests for Monday must be made by 4:00 p.m. EST on the previous Friday. You may ask questions or submit a request via telephone by calling the Prevention Services Division Monday-Friday from 8:00 a.m. – 4:00 p.m. at 317-347-5860 ext. 2105. 

Inspection requests submitted electronically or via telephone will receive a confirmation email.
