Request for Fire Incident Report

You may use this form to request a copy of a basic fire report. A basic fire report will contain information about the location of an incident response, actions of fire department personnel, specific loss information, and a fire cause determination (if applicable). The Pike Township Fire Department may have additional information about a specific incident available as part of the origin and cause investigation.

If more detailed information is being requested, call the Division of Prevention Services at 317-347-5860 ext. 2105. Please note that requests for additional information may require fees and extra processing time. Requests submitted for investigatory records are subject to denial per IC 5-14-3-1. Please allow at least 10 days from the date of an incident for a report to become available for release. After that time, please allow 5-7 business days for processing.
